● 研究目的日本血栓止血学会がLAスクリーニング検査の標準試薬として推奨しているPTT-LAの基礎的評価および他のLA検査結果との比較検討を行う。● 研究方法健常者32名からクエン酸ナトリウム加血漿を得て、PTT-LAを測定した。採血条件、遠心回数、血小板除去フィルター使用の有無による、正常値の算出を行った。さらに、KCT(180秒以上)の延長した患者検体14例(KCT-Mixing testにおいて陽性10例、保留2例、ワルファリン使用陰性2例)を用いてPTT-LA、KCT、APTT-SLA、dRVVT-におけるMixing testの結果を比較し、その乖離例の原因についても検討した。● 研究成果遠心条件2,000g、15分、20℃で処理した血漿でのPTT-LAの正常値は、1回遠心処理で33.7～44.2秒、2回遠心処理で33.5～45.1秒、1回遠心後フィルター処理で37.7～49.7秒であった。患者検体14例でのPTT-LAの結果は陽性5例、保留6例、陰性3例(うちワルファリン使用2例含む)となった。PTT-LA陽性例での一致率はKCT100%(5/5例)、APTT-SLA50%(2/4例)、dRVVT-screen20%(1/5例)であった。保留例での一致率はKCT33%(2/6例)、APTT-SLA100%(6/6例)、dRVVT-screen67%(4/6例)であった。陰性例での一致率はKCT67%(2/3例)、APTT-SLAとdRVVT-screenは100%(それぞれ2/2、3/3例)であった。PTT-LAでのLA陽性検体はすべてKCTでも陽性となったが、KCT陽性検体の中にはPTT-LAで陰性となる検体が1例見られた。しかし、この乖離例ついては検体残量が少なく原因究明には至らなかった。
