膠原病には間質性肺炎などの合併症があり、横断的研究では、膠原病の一つである全身性強皮症において間質性肺炎と運動耐容能との関連性、1回拍出量と肺機能や6分間歩行距離との相関が示されていた。本研究では、これまでの症例を縦断的に評価したところ、全身性強皮症では運動中の循環器応答は症例によって経過が異なり、心エコーで得られる測定値には関連するものがなく、影響因子として肺拡散能の追従性が良好で、6分間歩行距離とも関連していた。以上より、1回拍出量の減少は心機能自体の低下というより肺機能障害によって生じる体力低下が一因となっていると考えられた。
In previous cross-sectional studies, impaired stroke volume in patients with systemic sclerosis was reported, however no cardiac parameter using echocardiography was detected as the relating factor for the stroke volume reduction. In this study, the patients were re-evaluated, and correlations between the change of stroke volume for interval periods and cardiac parameters were analyzed.As the results, individual changes for the interval in stroke volume were found, which was not related to cardiac parameters, but was significantly related to the diffusion capacity of the lung for carbon monoxide. Significant relation was also found between the distance walked and the stroke volume. Thus, a limited lung diffusion capacity may affect stroke volume due to the exercise intolerance in systemic sclerosis without pathological heart involvement.
