【研究目的】MRSAは院内感染症の原因菌として最も分離頻度が高く、感染対策の観点から本菌の迅速な菌種同定と型別が求められている。当院では入院患者から分離されたMRSA株について、迅速にスペーサー領域のPCR泳動パターン(ITS-PCR)による型別と毒素産生性により菌株の識別を実施し、アウトブレイクの監視を行っている。しかし、同一病棟で同一パターンの株が分離された場合、さらに詳細な菌株識別が必要となる。そこで、今回Multiplex PCRを用いて短時間で容易に解析可能なPhage open reading flame typing(POT)方法による菌株識別について検討する。【研究方法】2011年4月から2012年3月に当院で分離されたMRSA105株について、ITS-PCR法と毒素産生試験(SET-A,B,C,D、TSST-1、EXT-A,B)、POT法(シカジーニアス分子疫学解析キット:関東化学)により菌株解析を実施した。【研究成果】(1)ITS-PCR法と毒素産生性:ITS-PCR法で13パターン、さらに毒素産生性を組み合わせると27パターンに分類された。ITS-Type22でSET-CとTSST-1産生(22-CT)株が21株(20.0%)と最も多く、次にType8でSET-A産生(8-A)株が18株(17.1%)、Type19でSET-CとTSST-1産生(19-CT)株が10株(9.5%)と3パターンで全体の46.7%を占めた。(2)POT法:47パターンのPOT型に分類され、POT1:93の値から62株(59.0%)が院内感染型(NY/Japanクローン)のSCCmecII型であった。ITS-PCR法で分離頻度が高かった22-CT株は7パターン(93-137-16,93-137-25,93-145-57,93-151-13,93-153-56,93-155-56,93-191-117)、8-A株は類似型の3パターン(106-183-32,106-183-37,106-183-45)、19-CT株は2パターン(93-19-39,93-222-47)のPOT型を示し、ITS-PCR法より詳細に分類することが可能であった。しかし、同一POT型を示した株の中にもITS-Typeや毒素産生性が異なった株や院内伝播の関連性が認められなかった株も存在し、今後の検討課題となった。【まとめ】MRSAの菌株識別は、ITS-PCRと毒素産生性による方法で迅速にスクリーニングを行い、アウトブレイクの可能性がある場合はさらにPOT法で詳細に識別することで、検査部から今まで以上に精度の高い菌株識別情報が発信でき、病院の感染対策および制御に貢献できると考えられる。
