手指屈筋群で手内筋のみが残存する2例と,屈筋群あるいは伸筋群の麻痺を伴う手指損傷患者5例の,計7例14手(年齢25.0±8.1歳)の示指MP関節の屈曲トルク値を,自作の測定器具を用いて測定し,健側手と対照させて,示指のMP関節屈曲における各筋の関与を検討した.症例1と2は,屈筋腱が再断裂し,人工腱挿入術後6カ月で,手指屈筋群は手内筋のみが残存しており,症例3と4は,術後6カ月で深指,浅指屈筋の回復が認められたが,手内筋の回復が見られない手内筋マイナス群で,症例5と6は,手内筋の回復も見られた手内筋プラス群である.また症例6と7は,術後3カ月と6カ月でMP関節屈曲トルク値を測定し,術後3カ月ではともに橈骨神経麻痺によりdrop fingerを示し,術後6カ月では麻痺は回復して指の伸展運動は可能であった.また全ての症例は,他動的に手指の関節可動域に制限はなかった.手内筋のみが残存する症例1と2の屈曲トルク値は,各々41.0%,39.2%で,手内筋マイナス群の症例3と4は,11.2%,19.1%,手内筋プラス群の症例5と6は,42.5%,27.8%であった.手内筋のみが残存する症例のMP関節屈曲トルク値は健側の約40%であり,主に第一背側骨間筋によるものと考えられた.また手内筋マイナス群はともにプラス群より小さく,これは,MP関節での手内筋による屈曲作用の低下とともに,DIP,PIP関節が屈曲位となり,MP関節での深指,浅指屈筋の屈曲作用の効率を低下させたためとも考えられた.また,橈骨神経麻痺を伴う症例6と7の術後3カ月では各々19.9%,32.1%,術後6カ月では27.8%,67.4%で,各々7.9%,35.3%増加していた.これより,伸筋群の筋力低下は,MP関節あるいはPIP関節での拮抗作用の低下を生じさせ,MP関節の屈曲力の低下に影響していると考えられた.
