Tissue factor pathway inhibitor(TFPI)は、活性型第X因子(FXa)と複合体を形成し、このFXaTFPI複合体が、活性型第VII因子(FVIIa)-組織因子(TF)複合体と反応することにより、外因系凝固機序を抑制する重要な外因系凝固インヒビターであることが近年明らかになった。TFPIの産生部位は内皮細胞と考えられているが、その存在形式として、血管内皮上のもの、リポ蛋白と結合したもの、遊離したものなど多様である。TFPIの各種病態における役割については不明な点が多く、播種性血管内凝固症候群(DIC)は極端な凝固活性化状態であり、TFPIの動態について検討することは意味深いと思われる。現在、血中のTFPI抗原量の測定は、総TFPI、遊離型TFPIの両者を測定することが可能となった(EIA)、各種疾患に合併したDIC症例における検討では、血中TFPIは健常人と比較して基礎疾患の存在のみでも上昇するが、DICの合併によりさらに上昇し(特に、急性前骨髄球性白血病、固形癌に合併したDICにおいて)、その上昇は線溶活性化の程度と併行した。この点をふまえ、DICの基礎疾患により(あるいは線溶活性化の程度により)TFPIの動態に差があることが予想され、少なくともLPS、TF、Tumor cells三種類のDICモデルにおける解析が必要であると考えられた。今後、腎などの臓器におけるTFPIの免疫染色を行うことにより、DICにおけるTFPIの病態にせまる予定である。
