対外衝撃波が日本白色家兎の大腿骨に微小骨折を生じさせ、骨形成を誘導することがこれまでの実験で明らかになってきた。成長期の骨端軟骨に対する影響を調べることは、今後の臨床応用を考えるにあたって必要と考えられた。日本白色家兎の大腿骨遠位端成長軟骨をモデルとし、対外衝撃波の成長に対する影響を調べた。[方法]生後7週令の日本白色家兎を実験に用いた。ケタラール(40mg/kg)の筋肉内投与後、耳介静脈よりのネンブタール持続投与により麻酔を行った。家兎8匹を4匹ずつの二群に分けた。A群は右大腿骨成長軟骨部に対外衝撃波を3shots、B群は6shots照射した。左側は、コントロールとした。対外衝撃波発生装置は、腎結石破砕装置の約3倍の圧力を発生できる、骨形成誘導用プロトタイプ(八千代田工業、東京)を用いた。照射前、照射後2、4、6、10週に大腿骨長のレントゲン計測を行い、左右を比較した。[結果]A群:照射前のコントロールは9.2±2.2cm、照射側は9.2±2.2cm、10週後のコントロールは9.3±3.1cm、照射側は9.6±2.4cmであった。10週後の左右差は統計学的に有意差(p=0.42)はなかった。B群:照射前のコントロールは9.1±1.4cm、照射側は9.1±1.3cm、10週後のコントロールは9.7±3.5cm、照射側は9.8±2.8cmであった。10週後の左右差は統計学的に有意差(p=0.39)はなかった。[考察]今回の検討では、衝撃波が成長を抑制するのか、促進するのかについての結論は得られなかった。照射数を増やせば、微小骨折を惹起するため、骨端線が閉じて、成長抑制が生じるのは想像に難くない。しかし、その手前の照射数でどう作用するかは不明のままである。今後は、犬を使うなどのモデルの変更等も考慮に入れねばなるまい。また、今後は偽関節モデル、脚延長モデルを作成し衝撃波の与える影響を調べる予定である。
