(目的)Tissue factor pathway inhibitor(TFPI)は、外因系凝固機序に対する生理的阻止因子であり、血管内皮結合型、リポ蛋白結合型、遊離型などの存在形式が知られている。まず我々は、播種性血管内凝固症候群(DIC)の患者を対象に、血中遊離型TFPI(fTFPI)の検討を行った。通常fTFPIは、血管内皮上のTFPIと平衡関係にあり、血中fTFPIの測定により間接的に血管内皮上のTFPIを推測できると考えられている。(対象、方法)対象は治療前DIC114例(I群)ヘパリン治療中(5000〜10000u/24hr持続、2〜3日経過)DIC22例(II群)、DICの基礎疾患を有するがDIC非合併の64例(III群)、健常成人24例(C群)である。(成績)血中fTFPIは、C群28.4±6.8ng/ml、III群45.8±19.6、I群97.6±76.1、II群140.3±126.1の順に高値であった。I群の内では固形癌、APLに合併したDICで、血中fTFPIは特に高値であった。血中fTFPIは、線溶活性化のマーカーであるプラスミン-α_2PI複合体(PIC)との間には有意の正相関がみられたが、凝固活性化のマーカーであるTAT、血管内皮上のマーカーであるTMとの相関はみられなかった。(結論)血中fTFPIは、DICの基礎疾患の存在のみでも上昇するが、DICの合併によりさらに上昇し、その上昇は線溶活性化の程度と平行すると考えられた。DICにおける血中fTFPIの上昇機序について(1)血管内皮上のTFPIが上昇しており、そのためfTFPIも上昇している可能性、(2)活性型酵素で分解されたいわゆるTruncated TFPIの増加の可能性が考えられ、この点についてひき続き動物DICモデルでの検討を行う予定である。
