経皮的血管内人工血管留置の臨床応用を目的としてCovered stent留置が第動脈壁に与える影響をBare stent留置の場合と比較した。covered stentはポリウレタン膜、シリコン膜、塩化ビニール膜を使用し作成した。雑種成犬20頭に対し15個のBare stentと13個のCovered stentを留置し経時的に犠牲屠殺した。検討方法として、大動脈造影像、組織標本、およびシリコンゴムによる栄養血管の透徹鋳型標本を用い、Covered stent留置部の開存性、大動脈壁の組織学的変化、大動脈壁栄養血管の変化について、Bare stent留置部の変化と比較検討した。全経過において高開存率を示したBare stent留置部に比して、covered stent留置部は種々の程度に狭窄ないしは閉塞を示し、その主たる原因は内腔の血栓形成と動脈壁と膜との間に生じた内膜肥厚であった。内膜肥厚の程度はbare stent留置ぶに比して有意に高度であった。これらは大動脈径と膜付stent径の不適合による膜の内腔側へのヒダ形成が強い部で高度であった。大動脈壁栄養血管は動脈壁全層および新生内膜で強い増生がみられ、その程度はBare stentと比較してより高度であった。以上の結果より、膜付きstent留置には強い血栓形成性、血管壁障害性、動脈壁栄養血管障害性がみられ、血管内膜付きstent留置に伴う問題点が明らかとなった。同様の検討を実際に臨床使用されている種々のStent,人工血管膜で行いいくつかの問題点が現在あきらかとなりつつある。
