• Note (General): 金沢大学医学部; 各種婦人科腫瘍について、多段階発癌に関与する遺伝子群、癌原遺伝子、癌抑制遺伝子群の変異を検索し、全症例の約80%にこれら遺伝子に何等かの変異が存在する事を明らかにした。即ち、Ras遺伝子の点突然変異は子宮体癌の前癌病変である異型増殖症で認められ、そのほとんどがk-rasコドン12、13の点突然変異で、すべてguanineからの変異であった。発癌過程における早期の変化であることが判明した。一方、癌抑制遺伝子のp53は異型増殖症には13例中1例(8%)と少なく、癌の進展に伴いGlで2/19(11%)、G2で1/7 (14%)、G3で6/14 (43%)と変異率は増加し、発癌過程での比較的晩期の変化であることが示唆された。突然変異の様式もすべてがguanineからの変化でコドン248に集中していることが判明した。他の癌抑制遺伝子DCCではD18S8における制限酵素Msp-IのRFLPを利用してLOHを検討したところ、1/7 (14%)に、m-RNAの低下は3/8例に認めた。Rb遺伝子のLOHも2/7 (29%)に、m-RNAの低下を1/7 (14%)に認めた。これらの多くは臨床進行期CIII期、あるいは未分化腺癌であった。さらに、人パピローマウイルス(HPV)がPCR法では6/47 (13%)に検出でき、約5%の子宮体部癌症例においてHPVゲノムが宿主DNAに組み込まれて存在することも明らかになった。ゲノムの不安定性やDNAミスマッチ修復遺伝子とくにhMSH-2の遺伝子異常についても約20%の症例に存在する事を明らかにした。以上をまとめると、子宮体癌におけるp53、K-ras、DCC, p16, RB遺伝子の異常やHPVの感染は高度分化型腺癌では17/26 (65%)、中程度分化型腺癌では5/7 (71%)、低分化型腺癌では12/13 (92%)に認められ、一部の症例では遺伝子異常が重複してみられた。しかし、単一遺伝子変異の率は前項で示すように低率であるため単一遺伝子をターゲットとする遺伝子治療は高率が悪いと考えられる。癌に普遍的に見られる変化として、不死化細胞のテロメア伸長に必要なテロメラーゼ活性の上昇が注目されている。我々も子宮体癌の90%近くにテロメラーゼ活性の上昇があることを突き止めた。この分子を標的とする遺伝子治療が有望であると考えられた。; Recent advances in molecular biology have allowed us to study cancer at the level of individual genes. Such works have led to the theory of multistep carcinogenesis.The present study showed that activation of oncogenes, inactivation of tumor-suppresor genes, inactivation of mismatched-DNA repair genes and infection of human papillomavirus were closely involved in the carcinogenesis of uterine endometrium. Alterations of Ras, p53, DCC,P16, C-kit/SCF,RB were totally found in 17/26 (65%) G1 adenocarcinmas, 5/7 (71%) G2 adenocarcinomas and 12/13 (92%) G3 adenocarcinomas. However, the each abnormality of a single gene is quite low in its incidence.A frequent genetic change which takes place in carcinogenesis should be choosed as the target molecule of gene therapy. Our modified non-Rl TRAP (telomeric repeat amplification protocol) assay found that the telomerase activation was common (more than 90%) in endometrial cancer and was a critical step in their pathogenesis.
