冠動脈バイパス術における代用血管として自己大伏在静脈に加え、近年使用されている自己動脈(内胸動脈、胃大網動脈)グラフトの血流供給能について、全身血行動態変化の影響、並びに種々の血管収縮剤および拡張剤に対する血管反応性の面から検討した。成犬を用い、左右内胸動脈を有茎グラフトとして剥離し、グラフト血管断端から自由流出するグラフト血流量を測定した。グラフト血流は血管拡張剤、特に塩酸パパベリンの血管内注入で著増した。心拍動下に右内胸動脈を右心耳に吻合した。ついで体外循環下に左内胸動脈を冠動脈左前下行枝に吻合し、吻合部中枢側の冠動脈を結紮した。超音波血流計を用いて、グラフト血流量の変化を測定した。血行動態安定時に左右内胸動脈グラフトの基準血流量を測定した。ついで血管収縮剤および拡張剤を投与することによって循環動態を変化させ、全身循環動態の指標とともにグラフト血流量の変化を測定した。右心耳に吻合した右内胸動脈グラフトは冠循環から独立しグラフト自体の血流供給能にのみ依存し、冠動脈に吻合した左内胸動脈グラフト血流は自己血流供給能とともに冠循環血流特性にも依存した。すなわち右心耳へのグラフト血流は収縮期優位であり、冠動脈へのグラフト血流は拡張期優位であった。循環血液量の減少や血管拡張剤の使用による低血圧によってグラフト血流は低下し、循環血液量の補充と血管収縮剤使用による血圧上昇によってグラフト血流は増加した。しかし、循環血液量不足時に血管収縮剤による昇圧を行うとグラフト血流は返って低下した。以上、冠動脈バイパスモデルを作成し、全身循環動態の影響と血管反応性の面から動脈グラフトの血流特性を究明した。
