- 2008-2010年度科学研究費補助金(基盤研究(C))研究成果報告書 課題番号:20592409 研究代表者:大牟禮治人 (鹿児島大学大学院医歯学総合研究科講師)これまでの研究から、食道内への酸逆流と睡眠時ブラキシズムとの関連が示唆されているが、その因果関係は解明されていない。本研究の目的は、睡眠時の食道内への酸刺激が睡眠時ブラキシズムを発現させるという仮説を実験的に検証することである。健康な成人男性12名を対象に咬筋筋電図や食道内pH計測を含む睡眠ポリグラフ検査を行い、睡眠中に酸性溶液を食道内に注入する介入を行った。その結果、酸注入後では筋電図バーストや睡眠時ブラキシズムエピソード、歯ぎしり音の頻度が有意に増加することが示された。また、酸が咽喉頭へ逆流していない場合にも睡眠時ブラキシズムが認められた。以上から、睡眠時ブラキシズムは食道内への酸注入により引き起こされること、酸の暴露範囲ではなく酸性度が睡眠時ブラキシズムのトリガーとなっている可能性が示唆された。Previous studies have suggested an association between sleep bruxism (SB) and gastroesophageal reflux; however, the relationship has never been characterized. To examine whether intra-esophageal acidification induces SB, we conducted a randomized trial to compare masticatory muscle activity after intraesophageal infusion with 5 ml acidic solution (0.1 N HCl, pH 1.2) and infusion with saline. Polysomnography with electromyogram (EMG) of the masseter muscle, audio-video recording, and pH monitoring of the esophagus were performed in 12 healthy adult males without SB. The frequencies of EMG bursts, rhythmic masticatory muscle activity (RMMA) episodes, grinding noise, and the RMMA/microarousal ratio were significantly higher in the 20-min period after acidic infusion than after saline infusion. These results suggest that RMMA episodes including SB were induced by esophageal acidification.
