各種癌はリンパ節転移を伴うことが知られており、リンパ節郭清を併施している。リンパ節転移の中にも微小な転移が存在し、微小転移陽性症例の予後が不良なこと、腫瘍から産生されるケモカインのよりリンパ管内を腫瘍が遊走しやすくなり、易転移の状況が形成されることが知られている。一方、多数の微小転移の形成が各種手技により報告されているが、その微小転移巣が必ずしも臨床上、顕性化しないことが予想される。我々はリンパ節微小転移モデルを作成し、微小転移の動向を基礎的・臨床的に観察し、抗腫瘍免疫の観点から考察を行った。腫瘍に発現したCD166およびCD168は周囲の間質と関係を持ちながら腫瘍の進展浸潤に有利に働き、胃癌の生物学的悪性度の指標となることを明らかにした。B17F1にリポフェクチン法でGFPを導入したマウスメラノーマ細胞を用いてリンパ節微小転移モデルの作成は可能で、リンパ節転移のみを形成できる実験系を確立したが、微小なGFPの検出が困難であり、次の研究の課題として実験継続中である。
We tried to clarify the association between chemokine expression and clinicopathological factors in gastrointestinal cancer, moreover we investigated how host immune function affect for micro lymph node metastases using mouse model. We showed the expression of CD166 and CD168 in gastric cancer positively correlated with clinical factors and was selected as prognostic marker in gastric cancer for the first time. We could establish micro lymph node metastases model using GFP transfected B17F1 and C57BL.
