- <p>The patient was a woman in her 70s. She died approximately 1 month after the onset of symptoms due to malignant lymphoma, which developed 15 years after undergoing liver transplantation. It was difficult to obtain tissue samples and establish a definitive diagnosis before her death; a diagnosis of diffuse large B-cell lymphoma was made during the pathological autopsy. EBV-DNA and soluble interleukin-2 receptor levels, which were elevated at the onset of the disease, were measured from stored serum samples collected up to 1 year prior to disease onset to assess their usefulness for early diagnosis. However, no significant elevation was observed before the onset of symptoms in either marker. The incidence of de novo malignancies after liver transplantation is considered high and may be associated with immunosuppressive therapy, although the exact mechanisms remain unclear. It is desirable to establish protocols for the adjustment of immunosuppressive drugs and for the screening of de novo malignancies.</p><p>症例は70歳代女性.肝移植後15年目に発症した悪性リンパ腫のために症状出現から約1カ月の経過で死亡退院となった.生前には観血的な組織採取は困難で確定診断には至らなかったが,病理解剖によりびまん性大細胞型B細胞性リンパ腫と診断された.発症時に上昇を認めたEBV-DNAと可溶性IL2レセプターについて,発症前1年間の保存血清を用いて早期診断への有用性を検討したが,いずれも発症前には有意な上昇は認めなかった.肝移植後のde novo悪性腫瘍は高率とされ,肝移植患者にとって重大な合併症の1つであり,原因として免疫抑制薬などの関連が示唆されるが詳細は不明である.de novo悪性腫瘍に対する取り組みとして免疫抑制薬の調整や癌スクリーニングのプロトコールの確立が望まれる.</p>
