助産領域において妊婦の冷え症は、分娩時異常のリスクであることは周知されている。しかしそれはあくまで経験知であり、エビデンスは欠如している。したがって、エビデンス確立のニーズは高く、冷え症に関する研究が求められている。本研究では、妊婦が冷え症であることが原因となり、その結果、異常分娩(早産･前期破水･微弱陣痛･遷延分娩･弛緩出血)が発症するという、因果効果の探索を行った。対象者数は分娩後で入院中の女性2810名である。分析方法は、共分散分析と層別解析であり、傾向スコアによる交絡因子の調整を行った。その結果、早産では、冷え症である妊婦の早産発生率の割合は、冷え症ではない妊婦に比べ、約3.4倍、前期破水では約1.7倍、微弱陣痛では約2倍、遷延分娩では約2.4倍であり、因果効果が推定できた(いずれもp<0.001)。弛緩出血では、冷え症の有無での明確な違いはなく、因果効果は推定できなかった。したがって、冷え症は分娩時のリスクであることが推定された。今後は、本研究結果を、医療者や妊婦ならびに女性に対して広くコンセンサスを得て、一般化していくことが喫緊の課題である。
