皮膚は我々の身体の外表を覆い、アレルゲンや病原体が身体の中に侵入するのを防いでいる。哺乳類皮膚表皮のバリアは、角質バリアとタイトジャンクションバリアの2つの要素から構成される。皮膚の最表層を覆う角質層のバリア機能不全が、慢性的な外来抗原の経皮的侵入の増加を招き、経皮感作が成立しやすくなることで、様々なアトピー疾患の発症要因となると考えられている。角質の内側では、タイトジャンクションのバリアによってアレルゲンや病原体の侵入を防ぎつつ、ランゲルハンス細胞がタイトジャンクションの外側から抗原を取得することで免疫をつけるというシステムが働いている。これらの精緻な仕組みの解明を行った。 Skin barrier is constituted by the stratum corneum barrier and tight junction barrier. Insufficiency of the stratum corneum barrier predispose patients to atopic dermatitis via enhanced percutaneous allergen penetration and subsequent allergic sensitization. We discovered that epidermal Langerhans cells penetrate through the tight junction barrier from inside to outside and take up antigens that penetrate through the stratum corneum but are limited to penetrate into the body by the tight junction barrier. In the atopic dermatitis patients, the tight junction penetration of Langerhans cells are increased in number, suggesting enhanced percutaneous sensitization against external allergens.
