医薬品が代謝を受け反応性代謝物が生成する「代謝活性化」は急性あるいは特異体質性毒性の原因となり、実際に代謝活性化による肝毒性のために使用が制限されたり市場から撤退した薬もある。本研究では、肝毒性を示す3つの医薬品 (ベンズブロマロン、ジクロフェナク、ネビラピン) に着目し、その代謝活性化機構に基づいて安全性の高い改良型医薬品を創製することを目指した。まず、研究に先立って反応性代謝物を定量的かつ効率的に検出できる新規蛍光標識トラッピング剤の開発に成功した。次に、各医薬品について代謝活性化を抑えた複数の誘導体を合成し、そのうちいくつかは薬効を維持した上で代謝活性化を回避できることが示された。 Metabolic activation of drugs to chemically reactive metabolites that are capable of modifying cellular macromolecules results in acute and/or idiosyncratic toxicities. Some marketed drugs are known to generate reactive metabolites and exhibit hepatotoxicity as side effects. These drugs must be carefully used on medication or withdrawn from the market in the worst-case. The current research is aiming to develop improved medicines to reduce the formation of reactive metabolites on the basis of the activation mechanism of three hapatotoxic drugs (benzbromarone, diclofenac, nevirapine). First of all, we succeeded in the development of a novel fluorescent trapping agent to quantitatively and effectively detect reactive metabolites. Next, we designed and chemically synthesized multiple derivatives of the three drugs to avoid their metabolic activation. It was found that some of the derivatives are not subject to metabolic activation with remaining pharmacological activities as expected.
