• Note (General): type:text; 本年度は研究計画最終年度として、医薬品開発から上市を経て継続的に臨床現場で継続的に使用されるための戦略を、承認申請にかかる時間（Drug lag）および市場におけるプレゼンスを"Life cycle management" という観点から検討した。特に、医薬品を商品としてみた場合、莫大な開発資金を効率的に回収し、次の新薬開発資金として運用することは、製薬企業においては最も重要な課題である。 Drug lagはdevelopment lag（特に日米の申請lag） とreview lagに分けることができるが、本年度は特にdevelopment lagに関して検討を加えた。2008年以降に承認された新薬のデータベースを作成し、日米の承認時期や、申請lagを検討した。その結果、日米間にはまだdevelopment lagは存在するが、新薬創出加算等のインセンティブの導入などにより、lagの短縮が見込まれる。 また、最近特に薬物治療の面で大きな進歩がみられる抗腫瘍薬のうち、特に市場に対するインパクトも大きいPD-1 阻害薬（PD-L1阻害薬含む）の上市からのlife cycle managementに関して検討を加えた。現在日本国内で要される5種のPD-1阻害薬の上市以降の販売数および売り上げを、経時的にデータベース化し、関連イベント（薬価改訂、適応拡大など）とのマッチングを行い、その影響に関してして検討を加えた。先発薬であるnivolumab の適応拡大による市場拡大と、薬価改定による売り上げの変動、日本における2番手のpenbrolizumab および他の3薬のの上市による市場状況の変動を検討した。医薬品のポテンシャルに加え、臨床試験の設計による適応承認が臨床現場における使用に大きく影響することが明確となった。今後の医薬品開発の主たるターゲットは、市場占有性の高さと、適応拡大の可能性が重要な因子となると考えられた。 なお、本研究の成果は薬学部で開講している「バイオ産業論」（全塾学生を履修可としている）で学生に紹介している。 We focus on the development drug lag between Japan and the United States, based on a database we created for all new drugs from 2008 to 2018 using publicly available data sources. We consider that innovative new drugs that are presumed to receive price premiums require enhanced policy support for early application from the initial stages of clinical trials. It is also essential to promote information sharing regarding evaluations by foreign reviewing authorities for efficient use in the home country. Another study we did in this period was to investigate the life cycle management for the drug in Japanese drug market. We specially focused our interest on the programmed cell death protein-1 (PD-1) inhibitors (Nivolumab, Pembrolizumab, Avelumab, Atezolizumab, Duravalumab). These drugs were considered to be useful for several cancer, therefore, the common development strategy was expanded adaptation.
