1.目的白血病は高率に播種性血管内凝固(DIC)を合併し、その凝固活性開始機序として組織因子(TF)/活性化凝固第VII因子(FVIIa)の関与が重要とされているが、その他の凝固開始機序として、白血球表面坑原であるMac-1(CD11)による凝固第X因子の活性化の関与も指摘されている。今回、まず、TF/FVIIaのDIC発病における役割を検討する目的で、基礎疾患として白血病を含むDIC例において、血中のTF坑原量、FVIIa値、そしてTF/FVIIaの阻止物質であるtissue factor-pathway inhibitor(TFPI)坑原量を測定した。2.対象対象はDIC27例で、その基礎疾患の内訳は急性前骨髄球性白血病2例、急性骨髄性白血病6例、慢性骨髄性白血病急性転化3例、非ホジキンリンパ腫2例、固形癌7例、敗血症4例、劇症肝炎1例、アナフイラキシ-ショック1例、膵炎1例である。なお、健常人20例をコントロールとして用いた。3.結果DIC例でのTF値は330.9±103.7pg/mlでコントロールの96.5±19.3pg/mlに比較して推計学的に有意に高値を示した(p=0.0001)。一方、FVIIa値はDIC例では47.3±30.0mU/ml、コントロールで47.4±18.5mU/mlと両群に有意な差を認めなかった。TFPI坑原量はDIC例で186.2±89.9ng/ml、コントロールで110.1±21.1ng/mlとDIC例で有意に高値を示した。(p=0.001)。4.考案今回の検討では血中TF坑原量はDIC例で高値を示したが、FVIIa値がDIC例で正常値を示したことについてはFVIIの活性化がDICで生じていない、TF/FVIIaの生成が局所で起こっているためFCIIa血中への流出が少ない、またFVIIaがTFPIにより即座に阻害されるなどの可能性が考えられた。
