【序文】
消化管病変の診断におけるゴールドスタンダードは内視鏡等で採取された検体の病理診断であるが，時に生検診断においては検体量や採取部位の問題から診断が困難な場合も経験される．また最近では画像強調を併用した拡大観察など，内視鏡による観察法の進歩により，病理診断に近い内視鏡診断が可能となっている．
しかしながら，そのような最新の内視鏡機器を用いても，すべての内視鏡医が正しく内視鏡診断ができるとは限らない．特に内視鏡の熟練医と初学医では，同じ病変を観察しても診断が異なることはまれではない．
熟練医は内視鏡で病変を観察しながら鑑別疾患を想定し，その疾患に特徴的な所見をとらえた撮影を行うのに対し，初学医はそのような鑑別疾患を想定していない撮影となり，検査後に別の内視鏡医がその写真を見ても診断が困難な場合がある．
近年は内視鏡に限らずさまざまな画像診断や検査機器の進歩により，医師が身体所見をもとに疾患を診断する機会は以前より減っている印象がある．しかしながら，身体所見が適切に判断されれば，過剰な検査を行わず正しい診断を下すことができるため，医療機器が発達した昨今においても，正しく身体所見をとることは医学教育において基本的診療能力と位置づけられている．それと同様に，内視鏡医の教育においても内視鏡所見からの鑑別診断の可否は内視鏡医の基本的診療能力として必須である．
