  • 概して臨床には，病歴・身体所見・検査値などの患者情報に基づいて病態を考え，鑑別診断を経て診断，そして治療とそのフォローアップ，という一連の過程があり，薬学生は実務実習で初めてそれを目の当たりにすることになります．これだけを聞けば，さほど真新しいことのない，ありふれた日常のように感じるかもしれませんが，実際の場に立ったとき，これがさほど簡単ではないことに気づかされるでしょう．「何年経ってもやはり臨床は簡単ではない」というのが正確な表現かもしれません．臨床はいつだって不確実性にあふれ，一筋縄ではいかないのです．

  • この簡単ではない実務実習に薬学生が臨むにあたり，どのように立ち向かうべきか．本書は，そんな学生さんたちの道標となることを期待しています．もちろん，大学で行う実務実習前の実習にも役立つでしょう．すでに現場で活躍されている薬剤師の先生方のお役に立てる内容も含まれていると思います．内容は決して簡単ではないかもしれませんが，張り切って学び，臨床で役立てましょう．

  • さて，本書は理論から実践までを含む内容となっています．構成は前半と後半で異なっています．

  • 前半は，臨床推論の理論的なお話です．Chapter 1の病歴と身体所見は，簡単なようで実際には難しいという最もギャップがある領域かもしれません．薬剤師は患者さんと対峙した際に会話をしますし，実際にその目で診ることができるわけですので，病歴と身体所見は薬剤師にもとれる重要な情報であることは間違いありません．そして，それらの患者情報をもとにどのように考えるのかがChapter 2 です．臨床で医療者たちがどのように考えているか，頭の中を覗いて見てみよう，というような内容になっています．こうした思考は診断プロセスにもかかわるため，医師と薬剤師の法的側面にも触れています．Chapter 3では理論から一歩踏み出し，だれしもが経験するであろう「風邪」と「痛み」に関する内容を扱います．学生であっても身の回りで身近にあろうこの症候・症状は，初学者が「考える」という過程を学ぶのに最も適した内容でしょう．そして，薬剤師となる以上，有害事象/ 副作用の学びは重要です．
