【刊行にあたって】
便失禁の原因として，加齢による肛門周囲の括約筋トーヌスの低下，出産や肛門手術によるもの，直腸脱などの肛門疾患に伴うもの，直腸癌手術後の排便機能障害など，様々な要因があります．近年，高齢化や自然肛門温存手術の普及により便失禁は増加してきていると推測されています．また，便失禁は生活の質を大きく低下させるため適切な診断と治療が望まれますが，これまで系統的にまとめられた指針はありませんでした．そのようななか，2017年にエビデンスを評価して推奨について系統的にまとめられた『便失禁診療ガイドライン』（初版） が発刊されました．
2024年版（改訂第2版） は，2017年版のエビデンスを整理し，新たなエビデンスを組み入れつつ，エキスパートにより繰り返し検討を重ねて作成されました．さらに作成委員会と評価委員会間でも検討がなされました．
本診療ガイドラインの目的は「便失禁診療・ケアを普及することで，便失禁症状を改善し，便失禁を有する患者の生活の質を改善する」 ことであります．そして，①便失禁の臨床評価，②特殊病態の臨床評価，③治療方針決定に必要な検査，④食事・生活・排便習慣指導の有用性，⑤薬物療法の適応と有用性，⑥骨盤底筋訓練・バイオフィードバック療法の適応と有用性，⑦洗腸療法の適応と有用性，⑧手術療法の適応と有用性の8つの重要臨床課題について明記して系統的に記述したうえで，5つのClinical Question（CQ） が設定され，臨床的な疑問に答えています．
排便の制御は，社会活動性や生活の質に影響を及ぼすのみならず，人格の尊厳に大きく影響します．超高齢社会である日本では，健康な日常生活をいかに保持するかが大きな課題となってきます．本ガイドラインの利用者としては，便失禁を診療する医師だけでなく，ケアを行う介護者や一般市民も想定されています．本ガイドラインが広く普及して多くの方にご利用いただき，便失禁診療の一助となることを切に望みます．
