「若手医師のための臨床に直結する診断学の良書」
医学生や研修医，そしてプライマリケア医の先生方にとって，脳神経疾患はとかく「複雑で難解」という印象をもたれがちで，その診断に苦手意識を抱く方も少なくありません．この壁を乗り越えるためには，患者さんの訴えをどう分類し，どこに疾患の原因があるのかを的確に理解することが求められます．本書は，そうした悩みに正面から応える構成と内容を備えた，まさに実地臨床に直結する「実践知の凝縮」とよぶべき一冊です．
本書では，神経診断に不可欠な「病因診断」と「病巣診断」という二つの軸が明確に提示されています．患者さんの訴えを適切な医学用語に置き換える作業から始まり，神経診察に基づいて責任病巣を推定し，さらに発症様式や経過を手がかりに病因を絞り込むという，脳神経内科医の思考プロセスが丁寧に解説されており，読み進めるうちにそれらを追体験できます．
総論では，「意識がおかしい」「うまく歩けない」「力が入らない」「めまいがする」「頭がいたい」「話し方がおかしい」など，日常診療で頻繁に遭遇する主訴ごとに，診察の際に注意すべき点，患者さんの訴えに耳を傾ける際のポイント，原因部位を考慮しながら行う診察のコツ，そして鑑別診断へと進む思考が体系的に示されています．とくに印象的なのは，単に症状を分類するだけでなく，「家族や発見者からの情報に耳を傾ける」「発症様式を見極める」といった，臨床現場に即した観察力や思考法への言及が随所に盛り込まれている点です．
