Alternative TitleThe clinicopathologic examination of local recurrence and secondary metastases to cervical lymph nodes factors in the tongue squamous cell carcinoma
Note (General)There are 81 cases, including 10 cases of local recurrence, and 20 cases of secondary metastases to cervical lymph nodes. Between recurrence cases and non-recurrence cases, p53,Ki-67,ALDH1 in the mucosal epithelium of the resection, the significant difference with the recurrence were not found, but showed a recurrence and significant correlation to degree of the cellular atypia and pattern of manifestation of podoplanin counts. Many cases of podoplanin counts showed significant correlation that a tumor showed infiltrative growth, but the case that caused secondary metastases to cervical lymph nodes did not show the tumor degree of differentiation, p53, Ki-67,ALDH1. As for the findings, the risk of the postoperative recurrence increasing in the case that cancer stem cells revealed manifestation of podoplanin that it was reported to develop to atypical epithelium cells for a recurrence, the of the tumor showed what the increase of podoplanin positive cells was associated with as well as the invasion activity of tumor cells, and it being with an index of one that it judged the prognoses such as a recurrence or the metastasis to determine manifestation of podoplanin as well as a search of the conventional histopathology immunohistologically was thought about.
【目的】口腔癌の術後再発や頸部リンパ節後発転移は患者の予後と密接な関係を有している. 舌扁平上皮癌早期症例を臨床病理学的に検索し, 局所再発と頸部リンパ節後発転移との関連性をもつ因子について検討した.【材料と方法】1988年から2012年までの25年間に北海道大学歯学部付属病院口腔外科および北海道大学病院口腔外科で根治切除を行った舌扁平上皮癌の中でStage 1, Stage 2の早期症例は98例だった. この98例中, 病理学的に後向き検索が可能であった81例を対象とし, 臨床病理学的な検索を行った. 検索症例の浸潤様式は戸塚らに従い浸潤型と膨張型に分類した. 摘出物の腫瘍に隣接する粘膜上皮を病理学的に検索し, 異型のみられたものは頭頸部癌取り扱い規約に準じてLow grade dysplasiaとHigh grade dysplasiaに分類した. さらにp53, Ki-67, ALDH1, Podoplaninに対する免疫染色を行い, 腫瘍細胞における陽性細胞率の計測と, 腫瘍に隣接する上皮におけるp53, Ki-67, Podoplanin, ALDH1陽性細胞率の計測とPodoplanin陽性細胞の分布について検討した.【結果と考察】対象とした81例中, 10例に局所再発, 20例に頸部リンパ節後発転移を認めた. 再発, 転移の両方を認めたものは2例あった. 頸部リンパ節後発転移をきたした症例では, 腫瘍が浸潤性増殖をしめすものとPodoplanin陽性細胞比率の高い症例で有意の相関を認めたが, p53, Ki-67, ALDH1陽性細胞比率との有意な相関は認めなかった. さらに膨張型増殖を示す腫瘍でもPodoplanin陽性細胞比率の高い症例は転移活性が高いことが示された. 再発症例と非再発症例間で, 腫瘍に隣接する上皮におけるp53, Ki-67, ALDH1陽性細胞比率と再発との有意な差は認められなかったが, 細胞異型の程度とPodoplaninの陽性細胞比率, 発現様式と再発との間に有意の相関を認め, 異型のないものやLow grade dysplasia症例でもPodoplanin陽性細胞率とPodoplanin Scoreとの間に有意な相関を認めた.以上の所見は, 頸部リンパ節後発転移は腫瘍細胞の浸潤活性に加えてPodoplanin陽性細胞の増加が関与していること, 腫瘍の再発には腫瘍に隣接する粘膜上皮の異型の有無および癌幹細胞で発現することが報告されているPodoplanin発現が術後再発の因子であることを示しており, 従来の病理組織学的検索に加えてPodoplanin発現について検索することが, 再発や転移の予測因子の指標となることが考えられた.
