Note (General): 現在日本人の約1300 万人は骨粗鬆症に罹患している。骨粗鬆症に起因する骨折は更なる骨折リスクの増大、寝たきり、そして死亡率の増加に繋がる。これにより医療費は増大する。一方、骨代謝および血管の石灰化は共通の病因を有するという証拠に基づいて、骨粗鬆症と動脈硬化症との関係が最近の研究で少しずつ解明されつつある。本研究ではパノラマＸ線写真により、動脈硬化症の最終段階である頚動脈石灰化と骨粗鬆症診断歴(既存骨折なし)および骨粗鬆症性骨折歴との関係について調査した。加えて、頚動脈石灰化が骨粗鬆症診断や骨粗鬆症性骨折の予測因子になりえるかについて検討した。研究対象は松本歯科大学病院を受診し歯科疾患の診断のためにパノラマＸ線写真を撮影した患者のうち、研究に同意の得られた40 歳以上1,021 名の日本人男女(男性371 名、女性650 名) とした。これら被験者に対して、既往歴( 骨粗鬆症を含む) と生活習慣についてアンケート調査を行った。パノラマＸ線写真上の頚動脈石灰化所見と骨粗鬆症診断歴および骨折歴との関係は二項ロジスティック回帰分析（ s tepwi se forward selec t ion）を用いてodds 比(95%信頼区間) にて評価した。頚動脈石灰化所見による骨粗鬆症診断歴と骨折歴のスクリーニング能力はReceive r Operat ing Charac t er i s t i c (ROC) 解析にて評価した。その結果、パノラマＸ線写真上で認められる頚動脈石灰化が、既存骨折のない骨粗鬆症診断歴と有意に関連していることが明らかとなった(odds rat io:1.82、95％信頼区間：1.02-3.24) 。しかしながら、骨粗鬆症性骨折は、パノラマＸ線写真上の頚動脈石灰化と関連しなかった(odds rat io:0.77 、95％信頼区間：0.33-1.77) 。ROC 曲線下面積は、骨粗鬆症診断歴で0.54、骨折歴で0.50 であった。以上の結果から、頚動脈石灰化所見は骨粗鬆症診断と関連は有するものの、頚動脈石灰化所見により骨粗鬆症リスクおよび骨粗鬆症性骨折リスク患者をスクリーニングすることは困難であることが示された。本研究で得られた結果は、歯科の病院を受診した患者に対してのみに限られた結果であるが、頚動脈石灰化所見と既存骨折のない骨粗鬆症診断との関連を示した世界で最初の研究である。日本人は欧米人に比して無症候性椎体骨折が多いため、一般住民集団において頚動脈石灰化所見が骨粗鬆症および椎体骨折とどのような関連を有するのかを明らかにすることが今後の課題である。
