Note (General): われわれはこれまで腫瘍血管内皮細胞には正常血管内皮細胞と比較して発現の高い分子が複数あることを報告してきた．その１つであるC-X-C chemokine receptor type 7(CXCR7)は腎癌など幾つかの癌の腫瘍血管において発現している．しかし口腔扁平上皮癌の血管内皮細胞におけるCXCR7の発現については未だ不明である．本研究では口腔癌のCXCR7発現を解析し，治療方針の決定に有用なバイオマーカーとしてCXCR7の有用性を探ることとした．1996年から2008年の間に北海道大学病院にて切除された口腔扁平上皮癌の組織検体のうち，HE染色，CD34免疫染色による解析が可能な59例を用いた．ランダムに選択した10視野からhot spot(CD34陽性血管が特に多い領域)を5視野選択し，連続切片でCXCR7抗体による免疫染色を行い，CXCR7陽性血管の割合を算出して評価した．腫瘍血管におけるCXCR7発現と臨床病理学的因子との関連を解析した．さらに，CXCR7陽性血管の割合の中央値をもとにCXCR7低発現群・高発現群の2群に分け，CXCR7の発現と予後との関連を解析した．癌部のCXCR7陽性血管の割合は41.9%，非癌部における陽性血管の割合は18.8%で，非癌部に比較し癌部の血管では有意にCXCR7発現の割合が高かった．また，腫瘍血管におけるCXCR7発現は，腫瘍径の大きさ(T分類)およびStage分類と正の相関が認められた．術後144ヶ月(12年)の全生存率は，CXCR7低発現群で95%，CXCR7高発現群で73%であり，また無病生存率は，CXCR7低発現群では70%，CXCR7高発現群では30%であった．腫瘍血管のCXCR7発現とこれらの予後には有意な差は認められなかったが，CXCR7高発現群では再発頻度が高い傾向が示された．腫瘍血管のCXCR7発現は患者の予後予測マーカーとして応用できる可能性が示唆された．
