Note (General): 心房細動は心不全や心原性塞栓症のリスク因子であり、加齢とともにその有病率が増加することから、超高齢社会を迎えた本邦の医療において問題となっている重要な疾患である。心房細動の薬物治療は、その治療域の狭さから、治療薬物モニタリングが実践されているが、治療薬物モニタリング指標に影響する因子が問題となる。本研究では心房細動に汎用されるジゴキシンとワルファリンを対象に、それらの治療薬物モニタリング指標であるジゴキシン血中濃度とプロトロンビン時間に着目した。特に心房細動は、心不全や弁膜疾患に合併することが多く、これらの治療薬による治療薬物モニタリング指標への影響が懸念される。したがって、その影響が明らかとなっていない薬物を対象に、近年国内で慢性心不全の適応を取得したエプレレノンと、弁膜疾患と重要な関わりをもつ感染性心内膜炎の第一選択薬に位置付けられるダプトマイシンに着目し、ジゴキシン血中濃度に対するエプレレノン、プロトロンビン時間に対するダプトマイシンの影響について定量的な評価を行った。その結果エプレレノンの臨床用量は、ジゴキシン血中濃度測定法であるfluorescence polarization immunoassay法、microparticle enzyme immunoassay法では臨床濃度で過大評価される危険性があること、affinity column-mediated immunoassay法、enzyme multiplied immunoassay technique法、chemiluminescent immunoassay法ではその危険性が低いことを明らかにした。一方、プロトロンビン時間においては、近年推奨されている高用量ダプトマイシン（> 6mg/kg）は、臨床上問題となるレベルで有意にプロトロンビン時間を偽延長させることを明らかにした。これらの知見は、心房細動治療薬で重要な治療薬物モニタリング指標である、ジゴキシン血中濃度およびプロトロンビン時間の評価の適正化に貢献できると考えられた。
