【背景と目的】難治性てんかんの治療法の一つに、皮膚電気活動（electrodermal activity: EDA）を用いたバイオフィードバック療法が存在する。これは、EDAを末梢交感神経機能の指標とし、バイオフィードバックによってEDAを亢進させることでてんかん発作抑制を目指す方法であり、他の治療法と同等の治療効果を持つとされる。しかしながら、その基盤となるてんかん患者のEDAに関する報告はなく、これらがどのような性質を持つのかは不明である。今回我々は、てんかん患者の安静時EDAを測定し、健常群と比較した。【対象と方法】てんかん患者22名と、健常者24名を対象に、横断的観察研究を行った。測定にはリストバンド型の皮膚電気活動測定器を用い、温度・湿度・時間などの測定条件を統一した上で、両側手首の安静時EDAを10分間計測した。またてんかん群においてEDAと、てんかんに関わる要因との相関を解析した。【結果】てんかん群と健常群の安静時EDAは、全10分間の平均において明らかな差を認めなかった。ただし測定開始後1分間において、てんかん群のEDAは健常群より低い傾向を認めた（P=0.12）。てんかん群から発作抑制者4名を除く18名で健常群と比較した場合は、有意にEDAが低かった（P=0.036）。また、てんかん群においては、発作頻度がEDAと有意に逆相関した（ρ=-0.50, P=0.016）。一方、罹病期間や薬剤数はEDAと相関しなかった。【考察および結論】てんかん群ではEDAが低く、また発作頻度が多いほどEDAが低下していた。これは、てんかん患者における交感神経機能低下を示していると考えられる。その機序としては、繰り返す発作が、交感神経機能を担う辺縁系-視床下部に機能不全を引き起している可能性がある。
