Note (General): 【緒言】Heparin-Binding Protein (HBP) は、敗血症患者の臓器障害を予測するバイオマーカーとして報告されている。本研究では HBP、インターロイキン (IL). -6、および IL-10 が集中治療および外傷の重症度スコアと相関するかどうかを評価し、感染を伴わない外傷患者の臓器障害を早期に予測し得るか検討した。【方法】2017 年 6 月から 2018 年 4 月の間に当院救命救急センターに入院した成人の外傷患者を対象とし、前向き観察単施設研究を行った。入院後 24 時間以内に、バイオマーカーとして、血中 HBP、IL6、IL-10 の濃度を同時に測定し、入院日 Acute Physiology and Chronic Health Score II (APACHEII). スコア、入院日 Sequential Organ Failure Assessment (SOFA) スコア、及び Injury Severity Score (ISS) との、相関係数と線形回帰係数を算出した。また、入院 2 日目の SOFA スコア＞5 点の場合を臓器障害有りと判定、各バイオマーカーによる臓器障害の予測能を評価した。【結果】40 例の患者が対象となった。HBP、IL-6、IL-10 と APACHE II、入院日 SOFA、ISS との間には有意な正の相関と回帰係数を認めた。臓器障害は 6/40 人に認め、HBP、IL-6、IL-10 の Receiver Operating Characteristic curve (ROC) の曲線下面積は、それぞれ 0.80、0.90、0.80 だった。HBPによる臓器障害の予測能は、カットオフ値 18.3 (ng/mL) で、感度 = 100％、特異度 = 64.7％だった。同様に、IL-6 はカットオフ値 42.9 (pg/mL) で、感度 = 100％、特異度 = 80.6％、IL-10 はカットオフ値 11.9 (pg/mL) で、感度 = 100%、特異度 = 70.6%だった。【結論】外傷患者において、入院 24 時間以内の HBP、IL-6、IL-10 と集中治療及び外傷の重症度スコア間に有意な正相関を認めた。更に、HBP や IL-6 の測定により、外傷患者の臓器障害の発生を早期に予測し得ることが示唆された。
