Note (General): 【目的】極低出生体重児について、未熟児網膜症の発症の有無と網膜光凝固の要否で 3 群に分けて、血漿中の成長因子、サイトカイン濃度を測定した。【方法】長岡赤十字病院新生児集中治療室に入室した 18 例を対象とした。児の健康状態が良好な場合に限り採血を行った。12 症例から検体を得、48 項目の multiplex assay を行った。修正在胎週数 33 週から 35 週での検体について群ごとに濃度を比較した。【結果】児の健康状態維持を最優先とするため採血を実施できた症例数および採血回数は少数に限られた。そのため統計解析は実施せず、各群間での傾向を把握することとした。採血時期が修正在胎週数 33 から 35 週の間の 9 症例で見ると肝成長因子 (Hepatocyte growth factor: HGF), interleukin -1 alpha; IL-1α、stromal cell-derived factor 1 - alpha;SDF-1α, stem cell growth factor - beta; SCGF-β の 4 項目で光凝固群では値が小さい傾向があった。【結論】血管成長因子とされる IL-1α が未熟児網膜症に関しては抑制的に働く可能性、網膜保護作用を有する肝成長因子や網膜の発生に不可欠な SDF-1 が不足することで未熟児網膜症が悪化する可能性が考えられるが、症例数の増加と経時的な検討を加える必要がある。
