Note (General): 【目的】口腔扁平上皮癌患者における制限拡散モデルパラメータを用いた拡散強調像についてFluorodeoxyglucose-PET (FDG-PET) 及びFluoromisonidazole-PET (FMISO-PET)のパラメータ， TNM 分類，Stage 分類との比較を行い，相関及び有意差について検討した．【材料と方法】28 人の口腔扁平上皮癌患者に対して前向き観察研究を行った．3 テスラのMRI 装置で6 通りのb 値(0-2500s/mm2)を用いてDWI 画像を作成し，計算した. 制限拡散モデルパラメータはK(kurtosis value), DK(kurtosiscorrected diffusion coefficient)，DDC(distributed diffusion coefficient)，α(diffusion heterogeneity)， Dslow(slow diffusion coefficient)，ADC(apparent diffusion coefficient)を計算した．SUV(standardized uptake values)はFDGPET及びFMISO-PET から，TMR(tumor-to-muscle ratio)はFMISO-PET から計算した．ピアソン相関係数及びWilcoxon の順位和検定にて評価した．【結果と考察】制限拡散モデルパラメータはFDG-PET SUVmax 及びTNM 分類との間に相関関係を示さなかった． FMISO-PET SUVmean とDslow との間に有意な相関がみられた．K，DK，DDC，ADC，Dslow とTMRmax 及びTMRmean との間に強い相関がみられた． K 及びADC とStage 分類との間に有意差がみられた．【結論】制限拡散モデルパラメータは腫瘍の低酸素状態を反映することから予後を予測できる可能性があると考えられた．
