Note (General): 【目的】足底部に誘発した急性炎症に伴う胃排出能の変化と,それらに対する鍼通電刺激の影響,さらにその機序についてカラゲニン炎症性痛覚過敏ラットを用いて検討した.【方法】SD 系雄性ラット 40 匹を用い,対照群(生理食塩水投与),カラゲニン投与群, カラゲニン投与+鍼通電刺激群と,カラゲニン投与+鍼通電刺激+ナロキソンメチオジド投与群の 4 群(各 n = 10)を比較した.胃排出能の計測は 20 時間の絶食の後,カラゲニン投与 4 時間後に 1.50g の餌を 10 分間で摂食させ,90 分後に胃を摘出し内容物を回収した.さらに 72 時間後に乾燥重量から胃排出能を算出し 4 群間を比較した.鍼通電は摂食前 1 時間,両側の足三里相当部位に 10Hz で行った.生理食塩水,カラゲニンの皮下投与,ナロキソンメチオジドの腹腔内投与の 4 時間経過後に,炎症所見として足部周径, 痛覚閾値および直腸温を計測し比較した.【結果】カラゲニン投与により足部周径の増大,直腸温の上昇,痛覚閾値の低下,胃排出能の遅延が確認された.これに対し,カラゲニン投与+鍼通電刺激群では痛覚閾値は上昇し対照群と同等となり,胃排出能も高くなった.さらにナロキソンメチオジドの投与を加えた群では痛覚閾値の改善は維持され,胃排出能遅延も対照群と同等まで改善した.【考察】鍼通電刺激による痛覚閾値の上昇には末梢オピオイドは関与せず,中枢性の機序の関与が示唆された.また,胃排出能遅延の改善には末梢オピオイドの関与は明確ではなかった.
