Note (General): 心臓突然死（sudden cardiac death: SCD）は心サルコイドーシス患者における主要な死亡原因であり、SCDの大部分は心室性不整脈が原因である。これまでのところ、心サルコイドーシス患者の致死的不整脈とSCDを予測するバイオマーカーは報告されていない。本研究では心サルコイドーシス患者における持続性心室頻拍（sustained ventricular tachycardia: sVT）およびSCDを予測する因子は何であるかを調査した。連続89症例において心サルコイドーシス患者の炎症活動性を反映する酸化的DNA損傷のマーカーである尿中8-hydroxy-2'-deoxyguanosine（尿中8-OHdG）、他のバイオマーカー、心機能の指標、腎機能を測定した。追跡期間中、15人の患者はsVT（N = 12）またはSCD（N= 3）を示した。COX比例ハザードモデルを用いた多変量解析では、尿中8-OHdG濃度および心室瘤（ventricular aneurysm: VA）の存在がsVT/SCDの独立した予測因子であることが示された。尿中8-OHdGおよびVAの存在は、心サルコイドーシス患者の初回のsVT/SCDの強力な予測因子であり、心臓イベントのリスクの層別化に有用である。さらに、これらはVT基質についてのさらなる情報を与えるものであることが示唆された。
