Note (General): 睡眠時ブラキシズム(Sleepbruxism:SB)の評価には，問診や口腔内所見などの臨床診断，小型筋電図による検査や睡眠ポリグラフ(PSG)検査に音声，ビデオ映像を組み合わせた検査法(PSG-AV)があり，現在のSB検査でのゴールドスタンダードはPSG-AVとされている[1，2]．PSG-AVは多くの情報を得ることができる一方で，操作性やコストの面，さらに慣れない環境下での検査であることから，臨床的な導入はされていない．筋電図単独のSB検査[3-5]では，患者が自宅でウェアラブル筋電計を皮膚に貼付してもらうことにより，とても簡便にSBの評価が可能となっている[6，7]．PSG-AVにおけるSB検査では，睡眠判定と脳波の記録，ビデオ映像により，中途覚醒時やSB以外の動作を除外することができ，より正確なSBの抽出が可能である．一方で，筋電図単独のSB検査では，中途覚醒時やSB以外の動作を波形から判断することは困難であり[8，9]，波形数のカウントにこれらの波形が含まれてしまい，実際のSB波形数より過大評価されるリスクがある[10，11]．そのため，筋電図単独でのSB検査の妥当性をPSG-AVと比較して検証した研究[4，12]では，高い正診率は示されてこなかったが，最新の研究では，筋電図単独の場合に適した波形数のカットオフ値を設定すれば，筋電図単独でもPSG-AVと同等の正診率を得られるとされている[7]．しかし，その研究では，筋電図単独検査の場合に就寝後や起床前のどのタイミングから筋電図波形の解析を行うのが望ましいかについての言及はなされていない．筋電図単独の検査でも，就寝や起床時刻は，患者の記録・申告により把握可能である[13]．しかし，入眠，覚醒時刻の正確な判定までは行うことはできない．そこで，筋電図単独で行うSB検査で，就寝後入眠するまでや覚醒後起床するまでの時間の筋電図記録をデータに反映させずに，純粋な睡眠時のデータのみを用いて解析するために，就寝，起床付近の時間帯を一律に除外するという方法が対策の一つとして考えられる[14，15]．しかし，一律に除外することの妥当性や，一律に除外する場合どの程度の時間を除外すれば十分なのかなど，就寝，覚醒時間帯付近の解析の取り扱いについての明確な基準は確立されていない．その理由としては，それらの時間帯の筋活動にターゲットを絞り，その実態を明らかにした報告がなかったことが挙げられる．そこで本研究では，就寝および起床付近の時間帯を解析に含めた場合や，一律に除外した場合の筋電図波形数と，実際の睡眠時間内の筋電図波形数を比較し，4筋電図単独でSBを評価する場合の適切な除外区間の解明を目的とした．
