• Note (General): 腰痛症は多くの人々が罹患する疾病であり，生涯罹患率が 8 割にも及ぶ．腰痛症の治療および予防の方法の一つとして，骨盤部に締付力を与える骨盤ベルトが広く用いられてきた．一方で，骨盤ベルトの長期使用には血行阻害や補助対象筋の萎縮などの悪影響も指摘されている．そこで本研究では，腰部負担に応じて締付力を自動的に調整する可変締付力制御により腰部を補助する骨盤ベルト型アシストスーツのアクティブコルセットを開発した．可変締付力制御において，適切な締付けの強さやタイミングといった特性や得られる補助効果は各個人で異なることが予想され，制御則を個人に適合させる必要がある．これらを決定するためには，補助効果の個人差が生じる要因を特定する必要がある．しかしながら，今日までに骨盤締付力により腰部負担が軽減されるメカニズムは解明されておらず，また補助効果の個人差がどのような要因から生じるのかは明らかにされていない．そこで本研究では，可変締付力制御則の開発に向けた締付力による負担軽減機序の解明および補助効果の個人差要因の特定を目的とする．補助メカニズムの検証では 93 名の被験者を動員した運動計測実験により前屈位における締付力の補助効果を検証した．力学モデルに基づき計算された腰椎関節トルクを腰部負担の指標として評価したところ，約 8〜9 割の被験者において腰部負担が軽減されることを確認した．このとき負担が軽減された被験者では腰椎関節の屈曲が抑制され，股関節により運動が代替される傾向が見られた．この運動矯正は前屈に関わる関節の屈曲に対する剛性が変化することで生じており，締付力が腰椎関節剛性を股関節剛性に対してより高い状態に変化させることが明らかとなった．さらに，X 線画像から得られた骨盤や腰椎部の姿勢や形状 (腰仙椎アライメント) を用い，関節剛性変化が腰仙椎アライメントの変形に起因することを示す回帰モデルを構築した．また，被験者が元々備える腰仙椎アライメントおよび体格的特徴から，関節剛性変化に対して高い寄与をもつ特徴量を特定し，補助効果を推定する回帰モデルを開発した．これにより骨盤ベルトの補助効果は骨盤に対する仙骨の姿勢角 (PI 角) や体重に強く影響されることが示された．得られた知見に基づき被験者の体格および腰仙椎アライメントを入力として締付力制御則を個人適合する手法を開発した．また，実用に向けて体格的特徴のみを入力に持つ手法を開発し，64 名の被検者を動員して個人適合された制御則の性能評価実験を行った．これにより提案法を適用することで過半数の被験者で補助効果が向上することを確認した．; Low back pain is a disease that affects a large number of people, with a lifetime incidence of approximately 80%. Pelvic belts have been widely used as a method of treatment and prevention of low back pain. On the other hand, long-term use of pelvic belts has been associated with negative effects such as impaired blood circulation and atrophy of the muscles. Therefore, this study developed the Active Corset, a pelvic belt-type power assist suit. Active Corset assists the lumbar region by using a variable tightening force control that automatically adjusts the tightening force according to the lumbar burden. In variable tightening force control, it is expected that there will be individual differences in characteristics such as appropriate tightening strength and timing, as well as assistance effects, and the control law should be personalized. To determine these factors, it is necessary to identify the factors that cause individual differences in assistance effects. However, the mechanism by which the pelvic tightening force reduces lumbar burden has not been elucidated, and it is not clear what factors cause the individual differences in the assistance effect. Therefore, the purpose of this study is to elucidate the mechanism of burden reduction by tightening force and to identify the individual difference factors of the assistance effect for the development of a variable tightening force control. The effect of the tightening force assistance in the forward bending position was verified by a motion measurement experiment with 93 subjects. The lumbar joint torque calculated based on the mechanical model was evaluated as the lumbar burden, and it was confirmed that the lumbar burden was reduced in approximately 80-90% of the volunteers. In volunteers with reduced burden, lumbar joint flexion was decreased and hip flexion tended to increase. Motion correction is caused by a change in joint stiffness, and it was found that the tightening force changes the lumbar joint stiffness to a higher value relative to the hip joint stiffness. Furthermore, the relationship between joint stiffness changes and lumbosacral vertebral alignment deformity was modeled using X-ray images. Also, a model was developed to estimate the assistance effect by identifying features with high contribution to joint stiffness change based on lumbosacral vertebral alignment and physique characteristics. The assistance effect was shown to be influenced by the posture angle of the sacrum relative to the pelvis (PI angle) and body weight. A method was developed to personalize tightening force control by using body size and lumbosacral alignment as inputs. In addition, 64 volunteers were mobilized for evaluation experiments of the control law, and it was confirmed that the assistance effect was improved in the majority of subjects.; (主査) 教授 田中 孝之, 教授 金井 理, 教授 山下 裕; 情報科学院(情報科学専攻)
