【目的】上腕屈筋群に遅発性筋痛を作成し，鍼の予防効果を検討した．【方法】研究には健常人 24 名（23.17±3.06 歳）を用い，コントロール群，局所（矩形波）群，遠隔（矩形波）群，遠隔（三角波）群の 4 群にランダム割付をした．研究対象者は，局所群では上腕二頭筋へ，また遠隔群では左右手三里―合谷にいずれも 3Hz15 分間の鍼通電刺激を行った後，上腕屈筋群に対して伸張性収縮運動を行った．評価は，運動負荷前，負荷後 0，24，48，72，168 時間後に，痛み（VAS とペインビジョン），肘関節屈曲・伸展時の疼痛誘発角度，上腕の体表温度・周径をそれぞれ測定した．【結果】コントロール群では全例で運動負荷直後から VAS に増加が見られ，負荷後 72 時間に最大になった．一方，痛みの評価に関しては各群で経時的な変化に差はないが，コントロール群と比べて疼痛誘発角度では局所（矩形波）群で有意な差が認められた（p＝0.04）．【考察】三角波は細径線維を選択的に刺激することで内因性オピオイドを放出し，炎症を抑えた可能性が，また局所の矩形波は局所血流や筋トーヌスを変化させることで，筋緊張が変化した可能性が考えられた．
