【序文】
思い起こせば，私をびまん性肺疾患への入り口への道しるべを示してくださったのは，今は亡き師匠の岩田猛邦先生である．
その師匠がびまん性肺疾患へ，歩んでいくことになったのは1978 年に外科的肺生検を行った典型的なびまん性汎細気管支炎（DPB）の症例であった．致死的な疾患であるとされたDPB のつながりで，当時奈良県立医科大学の教授であった三上理一郎先生が，師匠に声がけをしてくださり大阪のびまん性肺疾患研究会，厚生労働省班会議への参加への道が開かれていった．さらには，京都大学教授の泉孝英先生が主催された様々なびまん性肺疾患の研究会へ参画することになった．
そして，それらの会議に，鞄持ちとして同行していたことが，結局私がびまん性肺疾患への道を歩くことになった訳である．
師匠の臨床は，とにかく症例から何が学べるかという観点で診療していたことを強く教えられた．また，当時は鬼のように剖検をするというのが，岩田流でありそれがよい臨床，よい医療という観点で厳しく指導していただいた．さらに病気には原因があることを，常に口癖のように言っておられ，特発性はないんだと1980 年代から叫んでおられたことは忘れられない．実際その後に多くの病態が特発性から分離され明らかになっていったのは知っての通りである．ガイドライン主流である現在であるからこそ，原点に立ち返ってこの本を執筆するに至った訳であり，この書を岩田猛邦先生に捧げるとともに，若い先生方に思いが伝わることを切に願っている．
